To schedule your appointment, you can call into the office or send an email request.
Flexible scheduling is available to meet your needs.
Patients will receive a confirmation call for all appointments.
Appointment cancellations must occur at least 24 hours ahead of appointments.
You may be charged a cancellation fee for any appointments canceled in less than 24 hours.
(Please see the Financial Policy to review cancellations that occur on the same day.)
All new patients are required to bring the following to each appointment:
Arrive at least 15 mins before your appointment.
All DOT Physical Exams will be completed per FMCSA regulations.
Drivers should bring all items listed under new patients, in addition to their current DOT certificate.
If you wear glasses or use hearing equipment when driving, please bring to your appointment.

Our clinic uses Patient Ally as a patient portal. Patients are invited to register for access once they schedule their appointment. An invite will be sent to the patient via email.
The Patient Portal allows patients to view labs, request medication refills or appointments, and contact their provider via secure messaging.
Patients can contact the office if interested.
If you have forms that need to be completed or questions, please contact the office and schedule an appointment for completion or drop off forms.
There is a fee for forms completion of $20 that is due at the time the forms are presented to the provider.
All bills are due for payment upon receipt.
If a patient has an outstanding balance when an appointment is scheduled, he/she will be reminded of the amount due and asked to make payment before the scheduled appointment.
Payments may be made on arrival to appointments or by other methods per our Financial Policy before the appointment.
